For 2023, all forms are now online. To pay via mailed check, after you have filled the forms out:
- Email the form(s) to the email address specified on the form or FAX to (214) 461-0488
- Pay fees in one of two ways:
- Mail a check with the signed copy of your form (see #3) OR
- Pay online: CAAHF Payments Center
